Słuch

Problemy ze słuchem. Kolejny objaw w EDS - można stracić słuch całkowicie w każdym typie. W AGT była dziewczyna - piosenkarka i muzyczka - która z powodu EDS straciła słuch całkowicie.

U mnie pojawiają się objawy na razie delikatne, ale w najgorszych snach nigdy się ich nie spodziewałam. Jak siedzę tyłem do głośnika (w tej chwili działa już niestety tylko jeden) przy wyłączonym kompie, to nie słyszę buczenia. Jak delikatnie odwrócę głowę tak, że lewe ucho jest nakierowane na głośnik, to słyszę jego buczenie bardzo wyraźnie. :/ Prawym uchem tego nie słyszę. Podobnie kiedy śpię w nocy - na prawym boku nie słyszę, na lewym - buczenie mnie potrafi obudzić. Skoro mnie budzi, to nie jest jakieś delikatne i cichutkie. :(

Sprawa do zbadania, bo może aparat pomoże, ale nie stać mnie na kolejny medyczny wydatek, więc w najbliższych latach pewnie nie zrobię z tym nic. :(

Prawdę mówiąc, myślałam, że akurat ten objaw mnie ominie...

Sterylność

Od kilku miesięcy męczę się z odleżyną na prawym uchu. Nie będę się rozpisywać, jaki to absurd mieć odleżynę na uchu, ale to EDS, tu tak to wygląda.Duża nie była, ale ból był ogromny. Bolała mnie cała górna połowa małżowiny usznej, tak sama z siebie, bez dotykania. Nie mogłam też spać na prawym boku. Jak się może spać tylko na lewym bądź prawym boku, to w takiej sytuacji zostaje tylko jedna pozycja do spania - rozumiecie ten problem, prawda?

Jak zaczęłam stosować ilex, to odleżyna błyskawicznie, w ciągu kilku dni, wygoiła się na zewnątrz i zasklepiła. Z zewnątrz było tylko widać duże zaczerwienienie i ucho było gorące. Przez ponad miesiąc nic się nie zmieniało. Na zewnątrz wszystko wygojone, w środku toczył się stan zapalny, którego nie dawało się niczym ruszyć.

W końcu coś mnie tknęło i zastosowałam tribiotic, maść z antybiotykiem. I bingo! Już po pierwszym razie ból zmniejszył się tak, że mogłam spać na prawym boku! Myślę, że to jeszcze trochę potrwa, ale satysfakcjonuje mnie już ten mniejszy ból.

Ale pomyślcie, skąd się wzięły bakterie w odleżynie? Przecież ucho niczego nie dotyka! Tylko poduszki! I prawdę mówiąc, myślę, że to poduszka - od kilku miesięcy mam nową. Przyszła zapakowana w folię, więc... nie wyprałam poszewki.

Mieć odleżynę na uchu to samo w sobie lekko absurdalne, ale mieć w niej bakterie z powodu niesterylnej poduszki, to jest coś! Pokazuje, jak działa odporność w EDS6a. Ta choroba wymaga sterylności wszędzie - nawet na podłodze, po której chodzę boso - i co, jeśli będę miała na stopie choćby nieduże skaleczenie? Musi być sterylna wanna, ławeczka, sterylne ubranie. Praktycznie wszystko musi być sterylne, bo nie wiadomo, co gdzie złapię. :/

To praktycznie niemożliwe do uzyskania. :(

15 minut

Podciągnęłam się na fotelu, żeby być wyżej. Wtem! Skurcze! W palcach prawej stopy i lewej łydce - jednocześnie.
 
Zapuściłam muzykę i lekko podryguję na fotelu przy biurku. Nagle ostry, szeroko rozchodzący się tępy ból w obu łydkach.

To zakrzepica, ale nie mówi się o takich objawach. A to objawy raptem z ostatnich 15 minut.

Menopauza w ZA

Taki ważny temat, a tak pomijany. Tym trudniej to zrozumieć, że menopauza w EDS6a podbijana przez ZA bywa zagrożeniem życia. :(


"What do we know about the interplay between hormonal changes and autism for females*?
Our current understanding of how hormonal changes impact autistic individuals, and whether this differs from the impact for non-autistic individuals is still emerging. Because the female presentation of autism was unknown until recently, most research was conducted with autistic males. Thus, a particularly neglected area of research is the impact of hormonal transition on autistic females. Clinically, we have observed the devastation of the onset of puberty and menopause for our autistic females. Many autistic girls typically start to have friendship difficulties and more anxious meltdowns after they turn 9 years old, which is around the time that the social world becomes more difficult to navigate, but it is also a time of the beginning of hormone change. Globally, the median age of the recognition of autism for females coincides with the beginning of puberty, around 12 years old. We know that the onset of puberty is associated with, amongst other changes, a surge in anxious and depressive symptoms, as well as increased reactions to stress. It is unknown whether the hormonal changes are increasing the signs and features of autism, leading to a referral for a diagnostic assessment.

In our clinical practice, we commonly hear autistic women refer to the intense difficulties of menopause, another time of increased hormonal change and fluctuations. A recent qualitative study (Moseley et al, 2020) explored the experience of seven autistic individuals aged 49 to 63 years and found that the menopausal transition was associated with a number of difficulties. The three emerging themes from this research were: (1) a lack of knowledge and understanding amongst professionals; (2) increased autistic features, including high levels of anxiety, greater difficulties with social communication and relationships, executive functioning difficulties, and difficulties with sleep and self-care; and (3) difficulties finding help and support.

Two studies have found increased premenstrual symptoms for autistic women. Lever and Gertz (2016) reported that 21% of autistic women, compared with 3% of non-autistic women suffered from premenstrual dysphoric disorder (PMDD). Obaydi and Puri (2008) found that 92% of autistic women with learning disabilities experienced PMDD compared with 11% of non-autistic women with learning disabilities.

In a study this year, Groenman and colleagues (2022) did not find that PMDD was significantly higher in autistic women compared with non-autistic women (14.3% and 9.5% respectively) but did find that autistic women experienced increased menopausal symptoms. Interestingly, increased menopausal symptoms, such as hot flushes, poor sleep, increased anxiety, poor concentration, memory lapses (not uro-genital, eg vaginal dryness) were also found in non-autistic women who had ADHD compared to neurotypical women. Menopausal symptoms are known to be caused by fluctuations in oestrogen production. Experiencing more symptoms may mean that hormonal balance is different in neurodiverse women, however this has not been researched to date. Another possible explanation is that neurodiverse women may be more sensitive to their body during menopausal transition due to their overall increased sensory sensitivity, and thus experience menopausal symptoms more intensely.

We know that psychological ill-health, including increased levels of depression and anxiety, increases each of the diagnostic criteria of autism, specifically social communication difficulty, rigidity, difficulty tolerating change and transitions, different sensory processing systems, and theory of mind difficulties. Since menopause tends to decrease psychological health, it is likely that increased autistic features will occur. It is also the case that enjoying a better sleep quality is associated with less impact of menopause (Duffy et al, 2013), and many autistic adults suffer poorer sleep quality and quantity.

There may well be an interplay between hormonal levels, neurodiversity, poor psychological health, poor sleep, and sensory sensitivity that increase menopausal symptoms for neurodiverse women.

Where to from here?
More research is needed to further delineate the relationship between hormonal transitions and autism, particularly for autistic females. The emerging picture is that hormonal action that increases anxiety and depression, and interferes with a good sleep, will likely increase features of autism, leading to difficult menstruation periods, perimenopause and menopause.

First-line professionals including GPs and mental health professionals need to be aware of the potential negative impact of hormonal change and fluctuations on autistic women, to be able to provide appropriate understanding and supports.

We know that the impact of menopause is less for those who receive stronger social support, have greater physical fitness, have developed good coping strategies and who sleep well. Hence, increasing one’s social support network, incorporating body movement, learning new coping strategies for depression and stress and anxiety, and investing time in gaining good sleep are each likely to decrease the impact of menstruation and menopause.

It is worrying that during peri-menopause and menopause some autistic women experience significant decreases in their functional capacity, leading to less capacity for self-care and childcare. This difficulty needs to be recognised by funding bodies such as the NDIS, where the level of autism may become more severe during times of significant hormonal change.

Łamliwość kości w EDS6a

Niestety pacjenci z typem VI jak zwykle są bardziej narażeni na wszystko, także na złamania. To wyjaśnia, dlaczego po latach znajduje się w jakimś przypadkowym badaniu złamania, o których nie mieliśmy pojęcia. 

"Recent findings: Low bone mineral density and fractures seem to be frequent in some of the rare EDS types (kyphoscoliotic, arthrochalasia, spondylodysplastic, and classic-like EDS). For the more prevalent hypermobile and classic EDS types, some case-control studies found mildly decreased bone mineral density, but it was not clear that fracture rates were increased. Nevertheless, abnormalities in vertebral shape seem to be common in classical and hypermobile EDS types. In a cohort of individuals with EDS followed since birth, no fractures were observed during infancy. Bone mineral density varies widely among the different types of EDS, and vertebral abnormalities seem to be common in classical and hypermobile EDS. It might be justified to perform spine radiographs and bone mineral density assessments in newly diagnosed EDS".

Prosthetic Dislocation After Total Hip Arthroplasty

Abstract

Introduction:

Ehlers-Danlos syndromes (EDS) are genetic connective tissue disorders affecting multiple organ systems that frequently result in connective tissue hyperlaxity and early osteoarthritis. Short- and long-term outcomes after total hip arthroplasty in this cohort remain poorly characterized.
Methods:

Data were abstracted from the 2010 to 2018 PearlDiver Mariner administrative database. Patients with EDS undergoing total hip arthroplasty for osteoarthritis were identified and matched 1:10 to patients without a diagnosis of EDS. Ninety-day incidence of postoperative complications and hospital readmission were identified and compared. Odds of 90-day adverse events were compared using multivariate logistic regression. Finally, 5-year implant survival was compared using a log-rank test. Significance was set at P < 0.005 after Bonferroni correction.
Results:

Matching was done for 354 patients with EDS to 3,518 patients without EDS. Patients with EDS had markedly higher rates of periprosthetic dislocation within 90 days of surgery (4.2% dislocation rate in EDS vs. 1.7% in matched control subjects, P = 0.001). On multivariate analysis, patients with EDS continued to have increased odds of dislocation in the 90-day postoperative period (Odds Ratio=2.64, P = 0.001), whereas the rates of the other studied adverse events were not markedly different between groups. At 5 years, 92.7% of patients with EDS and 96.1% of the matched control subjects remained unrevised (significant log rank difference, P = 0.004).
Conclusion:

Compared with patients without EDS, patients with EDS were found to have a higher rate of perioperative dislocation and markedly lower implant survival at 5 years. These findings are consistent with the increased connective tissue laxity associated with EDS.

Diagnoza

 


Wciąż nie rozumiem, czemu lekarze, którzy nie zdiagnozowali EDS6 u chorych, nadal wykonują ten zawód i nie ponieśli żadnej kary. Krzywdzą kolejnych pacjentów i zagrażają ich życiu. Nie wspominając o tym, że jeszcze im wmawiają, że sobie wymyślili te objawy. Powodują medyczne PTSD, depresję i masę innych zaburzeń psychicznych, a potem to na nie zwalają objawy choroby podstawowej, której nie chcą zdiagnozować. Ilu chorych tego nie przeżyło? I żadnej kary nadal dla takich lekarzy nie ma!

20 lat - trzeba dodać! - to super czas. Tak szybko niestety nie diagnozuje się choroby ultrarzadkiej. Większość osób z EDS6a umiera bez diagnozy.

Kolagen

 


Tak wygląda kolagen w moim ciele, w ciele edesiaka. Nie tylko te pomocnicze typu II i V (jak w innych typach), ale przede wszystkim kolagen główny typu I, ten, który jest podstawowym budulcem WSZYSTKIEGO w ludzkim organizmie.
 
Jakim cudem to ciało się jeszcze nie rozpadło - nie wiem, ale chyba teraz już się nikt nie powinien dziwić, że pacjenci umierają na wszystko. :(

Costochondritis

 


[Tu więcej]. Zbyt intensywny wysiłek to np. oddychanie. I oczywiście nie leczy się tego żadnym paracetamolem, tylko proloterapią - ostrzykuje się chore miejsce volonem. Inne sterydy dostępne w Polsce nie działają w eds6a. NIGDYnie ustępuje samoistnie. I jak raz się przypałęta, to potem wraca regularnie do końca życia, tak jak zapalenia ścięgien.

Niestety nie wolno tego ignorować, żeby nie przeoczyć kiedyś prawdziwego zawału serca. :(
